If I have 2 sites with gatesways should they have the same AE Title so that the images show on the same worklist.

Typically you want use different AE titles for each site so the cases appear on your worklist under the appropriate source facility. You can independently determine which facilities are displayed on your users' worklist by granting or denying facility privileges in Admin -> Users.
